Male Fertility Testing: 6 Things a Semen Analysis Misses When You’re Trying to Conceive [EP 77]
Male fertility is still one of the biggest blind spots in fertility care. Male factor infertility contributes to roughly 40–50% of infertility cases, yet I continue to see couples spend months or even years investigating the female partner while the male partner gets a basic semen analysis and little else.
A semen analysis is important, but it is not a comprehensive assessment of male fertility. Sperm quality matters not only for getting pregnant, but for embryo development, staying pregnant, and supporting a healthy pregnancy because sperm contributes 50% of the DNA required to create an embryo.
In this episode, I'm breaking down six areas that deserve a closer look when we're evaluating male fertility, from sperm DNA fragmentation and hormones to nutrient status, diet, oxidative stress, and environmental factors. More importantly, I'll walk you through what you can actually do with that information so you can stop treating male fertility as an afterthought and start investigating both partners from the beginning.

Male fertility is still one of the biggest blind spots in fertility care.
Male factor infertility contributes to roughly 40–50% of infertility cases. In other words, in about half of couples struggling to conceive, the male partner is contributing to the fertility picture in some way.
Yet I continue to see couples spend months or even years investigating the female partner while the male partner gets a semen analysis and little else.
If the semen analysis comes back within the reference range, he's often told that everything looks fine.
But a semen analysis is not a comprehensive assessment of male fertility.
It does give us important information, but it does not tell us the whole story.
This becomes even more important when a couple has experienced recurrent pregnancy loss, failed IUIs, failed IVF cycles, poor fertilization, poor embryo development, or unexplained infertility.
Sperm contributes 50% of the DNA required to create an embryo.
So sperm quality isn't only important for getting pregnant. It is also important for embryo development, staying pregnant, and supporting a healthy pregnancy.
Today, I want to walk you through six areas that deserve a closer look when we're evaluating male fertility and, more importantly, what you can actually do with that information.
A Better Look at the Semen Analysis
We don't want to skip the basics.
A semen analysis does matter, and we do want to evaluate the parameters. But we need to make sure we're getting the full picture.
Rather than simply hearing that everything is "normal," I want you to actually look at the numbers.
We want to evaluate:
Sperm concentration
Total sperm count
Motility
Progressive motility
Morphology
Semen volume
Total motile sperm count
And that's the minimum.
One number alone does not determine whether someone is fertile or infertile.
Semen parameters can also fluctuate, which is one reason repeating an abnormal analysis can be valuable.
But there's something else that's important to understand.
A semen analysis tells us what the sperm looks like and how it behaves.
It does not tell us whether the DNA inside the sperm is healthy.
That brings me to one of the biggest pieces I think is overlooked: sperm DNA fragmentation.
Sperm DNA Fragmentation
Sperm DNA fragmentation is particularly important in couples experiencing recurrent pregnancy loss, but it can also be valuable when a couple is simply taking longer to conceive.
Sperm can look relatively normal on a conventional semen analysis while still carrying significant DNA damage.
We now have stronger evidence connecting elevated sperm DNA fragmentation with longer time to pregnancy and miscarriage.
If you've experienced recurrent pregnancy loss and have repeatedly been told that it is probably an egg quality issue because of the female partner's age, I want you to push back and advocate for yourself.
Egg quality matters, of course.
But sperm quality matters tremendously too.
DNA fragmentation can be influenced by oxidative stress, smoking, environmental exposures, varicocele, illness and fever, paternal age, and other factors.
This is why I want couples to stop thinking of sperm testing as simply checking whether there are enough sperm swimming in the right direction.
We need healthy DNA inside the sperm too.
If you've experienced recurrent pregnancy loss, unexplained infertility, poor embryo development, or repeated fertility treatment failure, talk with a reproductive urologist about whether sperm DNA testing is appropriate for you.
DNA fragmentation can be changed.
I've seen DNA fragmentation improve from 64% to optimal levels below 20% within about five months, followed by a healthy pregnancy and baby.
It's absolutely possible to improve sperm quality, and I often see improvements much sooner than that.
Hormones and Metabolic Health
We also need to think about hormones and metabolic health.
When testosterone is low, I don't want the conversation to immediately become, "How do we increase testosterone?"
I want to know why testosterone is low.
Testosterone production does not happen in isolation.
Male reproductive hormones communicate through the hypothalamic-pituitary-gonadal axis.
Depending on the situation, a more complete hormonal evaluation may include:
Total testosterone
Free testosterone
FSH
LH
Estradiol
SHBG
Prolactin
Thyroid markers
We also want to understand what's happening with fasting glucose and insulin, body composition, sleep quality, snoring, and potentially sleep apnea.
Other factors matter too, including alcohol intake, medications, stress load, and whether someone is chronically under-recovered from over-exercising or not getting enough sleep.
Low testosterone can be a clue that something bigger is happening physiologically.
I want to address those underlying factors rather than simply chasing the testosterone number.
And something extremely important: if you're trying to conceive, do not start testosterone replacement therapy without discussing fertility with your healthcare team.
Testosterone replacement can suppress sperm production and can effectively act like birth control for men.
If testosterone is low, ask why.
Evaluate the broader hormonal and metabolic picture before jumping into anything designed to increase testosterone.
Micronutrient Status
Sperm production is incredibly nutrient-intensive.
You're producing millions of sperm every day, and those cells are particularly vulnerable to oxidative stress.
Two nutrients I pay extra attention to are zinc and vitamin D.
Zinc plays an important role in male reproductive health, including sperm development and function.
But nutrients work together.
If you're supplementing zinc aggressively for long periods of time, you can disrupt copper balance.
So don't blindly supplement, even with something like zinc that can be very helpful.
We don't want to throw off copper or iron balance because those nutrients are also important for overall sperm health and metabolic health.
We want to understand you as an individual rather than blindly using high-dose supplements.
Vitamin D is another nutrient worth assessing.
Research on supplementation is somewhat mixed, but vitamin D status has been associated with sperm motility and morphology.
That doesn't mean more vitamin D is better.
It means deficiency deserves attention.
Before building a large male fertility supplement stack, look at your individual nutrient levels, your diet, and your history and symptoms.
The goal is to correct deficiencies rather than blindly taking handfuls of supplements.
Dietary Fats and Omega-3s
We also want to think about dietary fats.
Omega-3s and fatty fish are not only important for female fertility. They're foundational for sperm health too.
DHA is highly concentrated in sperm cell membranes and is involved in sperm structure and function.
Research has also found associations between higher fish consumption and higher sperm counts and improved sperm morphology.
One of my favorite recommendations is incredibly simple:
Aim for two to three servings of fish per week.
If you can include fatty fish, that's especially supportive for omega-3 intake.
Think:
Salmon
Sardines
Anchovies
Trout
Cod
Halibut
The less fatty fish are also excellent sources of protein and can support an overall nutrient-dense diet.
Antioxidants and Oxidative Stress
This is the piece that brings everything together.
We spend so much time looking at sperm after they've been produced.
I also want to look at the environment where they're actually being produced.
Sperm cells are particularly susceptible to oxidative damage.
That means we need to consider the balance between oxidative stress and antioxidant defense.
Food is an important place to start.
Carotenoid-rich foods are an easy addition.
Think tomatoes, watermelon, red peppers, carrots, sweet potatoes, citrus, red and orange produce, and dark leafy greens.
Lycopene is a carotenoid found particularly in red and orange foods and has been studied for male fertility because of its antioxidant properties.
Overall, increased lycopene intake has been associated with improvements in sperm health, including morphology.
I want you to build an antioxidant-rich diet because sperm needs protection from oxidative stress.
But food is only part of the environment.
Lifestyle and Environmental Factors That Affect Sperm
If you want to support sperm health, we also need to look at the exposures happening every day.
This means:
Stop smoking and vaping
Avoid marijuana
Avoid heavy alcohol intake
Minimize excessive heat exposure
Avoid hot tubs and saunas while trying to conceive
Don't place a laptop directly on your lap
Don't keep your phone in your front pocket
Minimize exposure to pesticides, solvents, and other chemicals
Prioritize seven or more hours of sleep
Reduce processed foods
Focus on whole foods
Manage blood sugar and insulin response
Evaluate for varicocele when appropriate
Getting less than six hours of sleep is another factor to take seriously.
Ideally, you're getting at least seven hours of sleep every night.
Sometimes the answer isn't another supplement.
Often, it's not.
Sometimes there are underlying medical or anatomical issues that deserve evaluation.
A Comprehensive Approach to Male Fertility
This is one of the biggest messages I want you to take away from today's episode:
Sperm quality is not just a fertility issue.
It can be a window into overall male health.
If you're listening to this and thinking, "We haven't done any of this," please don't panic.
I want you to see the opportunity here.
Male fertility is not something we should wait to investigate after we've exhausted everything on the female side.
We should be investigating both partners from the very beginning.
If I were building a more comprehensive male fertility assessment, I would want to consider:
A detailed semen analysis
Sperm DNA fragmentation
A thoughtful hormone evaluation
Metabolic health, including blood sugar and insulin when appropriate
Nutrient status and dietary adequacy
Lifestyle and environmental factors
Physical factors that may influence sperm production
Then we use that information to build the plan.
The goal isn't simply to get better numbers on the next semen analysis.
The goal is to create healthier sperm with better potential to fertilize an egg, contribute healthy DNA to an embryo, and support a healthy pregnancy.
How Long Does It Take to Improve Sperm Quality?
Remember, sperm development takes time.
If the sperm being ejaculated today started developing weeks ago, we're really talking about months.
A full spermatogenesis cycle is at least 75 days.
That means the changes you make now matter for the sperm you'll be working with over the next few months.
Start with the fundamentals.
Eat enough protein and nutrient-dense whole foods.
Get colorful plants on your plate every day.
Eat two to three servings of fish each week.
Make sure your omega-3 intake is adequate.
Correct nutrient deficiencies rather than blindly supplementing.
Prioritize sleep and recovery.
And if something is abnormal, keep asking why.
Male factor infertility is not the end of the investigation.
It's the beginning of understanding what may be interfering with healthy sperm production.
Fertility Is a Couple's Health Issue
